Como cada d\u00eda, suena el despertador. Pero hoy conmigo se despiertan tambi\u00e9n la primavera y el florecer de los \u00e1rboles, arbustos y dem\u00e1s plantas que fabrican esa sustancia tremendamente vol\u00e1til y ligera llamada polen. <!--more-->Cuando llega a mi nariz, el polvillo invisible me produce mucosidad, irritaci\u00f3n ocular y ganas de estornudar. Me dirijo al cuarto de ba\u00f1o, abro el armario del botiqu\u00edn y, \u00a1<em>voil\u00e1<\/em>!, aparece mi salvadora: una cajita blanca con unas letras que dicen \u201cAtarax\u201d. La abro, tomo una pastillita y a trabajar. Pero tendr\u00e9 que beber un caf\u00e9 de m\u00e1s antes de la reuni\u00f3n ejecutiva, ya que el antihistam\u00ednico que contienen estos comprimidos me salvar\u00e1 del brote al\u00e9rgico, pero tambi\u00e9n me provocar\u00e1 un sue\u00f1o que dif\u00edcilmente podr\u00e9 disimular delante de mi jefe.\u00bb<\/p>\n<p style=\"text-align: justify;\">Si es usted al\u00e9rgico al polen, esta escena le resultar\u00e1 familiar. Cuando ingerimos un antihistam\u00ednico, este pasa al sistema digestivo, se absorbe y llega al riego sangu\u00edneo, donde se distribuye, de forma generalizada, a lo largo y ancho del cuerpo. Finalmente, alcanza los receptores de histamina que nos provocan las dichosas reacciones al\u00e9rgicas y los inactiva, con lo que desaparecen \u00abmilagrosamente\u00bb los molestos s\u00edntomas. No obstante, en este recorrido, una peque\u00f1a fracci\u00f3n del f\u00e1rmaco llega al cerebro, donde tambi\u00e9n hay receptores de histamina, y produce ese sue\u00f1o inexcusable. Un caso t\u00edpico de efecto secundario.<\/p>\n<p style=\"text-align: justify;\">Los antihistam\u00ednicos ilustran lo que suele ocurrir con la mayor\u00eda de los medicamentos: que se distribuyen por el cuerpo de forma indiscriminada. Si a ello le sumamos el hecho de que, en numerosas ocasiones, la prote\u00edna o receptor para el que el f\u00e1rmaco ha sido dise\u00f1ado (diana) desempe\u00f1a funciones distintas seg\u00fan donde se encuentre en el organismo, el efecto secundario est\u00e1 servido. As\u00ed, el f\u00e1rmaco puede provocar el efecto deseado en una parte del cuerpo (antial\u00e9rgico, en el ejemplo que nos ocupa), mientras que en otra regi\u00f3n produce consecuencias no deseadas (somnolencia). Lo que falla, pues, es lo que los farmac\u00f3logos denominan \u00abselectividad del lugar de acci\u00f3n\u00bb. (Si bien en el caso de los antiestam\u00ednicos parte de este problema se ha solucionado con f\u00e1rmacos de segunda generaci\u00f3n que atraviesan en menor medida la barrera hematoencef\u00e1lica, estos siguen provocando efectos secundarios por su actividad en regiones distintas al sistema nervioso.)<\/p>\n<p style=\"text-align: justify;\">Adem\u00e1s de la falta de selectividad en el lugar de acci\u00f3n, los efectos secundarios pueden deberse tambi\u00e9n a otro factor: la falta de selectividad del f\u00e1rmaco para activar o desactivar solamente cierto tipo de prote\u00ednas o receptores.<\/p>\n<p style=\"text-align: justify;\">Imagine ahora que tenemos en nuestras manos un medicamento que podemos introducir en el cuerpo humano y dejar inactivo hasta que nosotros le indiquemos cu\u00e1ndo, d\u00f3nde y de qu\u00e9 manera debe actuar. Imagine tambi\u00e9n que utilizamos la luz para activar y desactivar este f\u00e1rmaco en la parte del cuerpo que queramos, durante el tiempo que necesitemos y con la intensidad que nos interese. Esto es precisamente lo que persigue la fotofarmacolog\u00eda, una rama emergente de la ciencia que desarrolla terapias basadas en f\u00e1rmacos fotosensibles.<\/p>\n<p style=\"text-align: justify;\">La fotofarmacolog\u00eda se enmarca en el campo de la administraci\u00f3n dirigida de f\u00e1rmacos (<em>targeted drug delivery<\/em>), una estrategia que pretende solventar los problemas derivados de la falta de selectividad de los medicamentos. En este gran grupo de t\u00e9cnicas, generalmente focalizadas en encontrar nuevos tratamientos contra el c\u00e1ncer, se incluyen tambi\u00e9n el encapsulamiento de f\u00e1rmacos en nanopart\u00edculas, lipomas, micelas o dendr\u00edmeros que liberan el medicamento de modo m\u00e1s o menos preciso en el tejido que se debe tratar.<\/p>\n<p style=\"text-align: justify;\"><strong>Fuente:\u00a0<\/strong><\/p>\n<p style=\"text-align: justify;\"><em>Xavier G\u00f3mez Santacana, Xavier Rovira Algans, X.
