{"id":28093,"date":"2022-10-06T08:57:24","date_gmt":"2022-10-06T14:57:24","guid":{"rendered":"https:\/\/otech.uaeh.edu.mx\/noti\/?p=28093"},"modified":"2022-10-06T08:57:24","modified_gmt":"2022-10-06T14:57:24","slug":"nueva-combinacion-eficaz-de-antibioticos-contra-bacterias-multirresistentes","status":"publish","type":"post","link":"https:\/\/otech.uaeh.edu.mx\/noti\/ciencia\/nueva-combinacion-eficaz-de-antibioticos-contra-bacterias-multirresistentes\/","title":{"rendered":"Nueva combinaci\u00f3n eficaz de antibi\u00f3ticos contra bacterias multirresistentes"},"content":{"rendered":"<p style=\"text-align: justify;\"><span style=\"color: #000000;\">La creciente tasa de infecciones debidas a microorganismos resistentes a los antibi\u00f3ticos es un importante problema de salud p\u00fablica y ha dado lugar a serios desaf\u00edos terap\u00e9uticos en el entorno cl\u00ednico.<!--more--><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Dos investigaciones sobre este tema, realizadas por varios grupos del Centro de Investigaci\u00f3n Biom\u00e9dica en Red de Enfermedades Infecciosas (<a style=\"color: #000000;\" href=\"https:\/\/www.ciberinfec.es\/\" target=\"_blank\" rel=\"noopener noreferrer\">CIBERINFEC<\/a>) en Espa\u00f1a, se han centrado en estudiarla sensibilidad antibi\u00f3tica, la epidemiolog\u00eda y el resistoma de aislados multirresistentes de enterobacterias y Pseudomonas aeruginosa, centr\u00e1ndose en los mecanismos de resistencia a la nueva combinaci\u00f3n antibi\u00f3tica imipenem-relebactam.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Los trabajos, publicados\u00a0en las revistas acad\u00e9micas<a style=\"color: #000000;\" href=\"https:\/\/journals.asm.org\/journal\/spectrum\" target=\"_blank\" rel=\"noopener noreferrer\">\u00a0Microbiology Spectrum<\/a>\u00a0y<a style=\"color: #000000;\" href=\"https:\/\/academic.oup.com\/jac\/issue\/77\/10\" target=\"_blank\" rel=\"noopener noreferrer\">\u00a0J Antimicrob Chemother<\/a>, forman parte de dos estudios multic\u00e9ntricos de vigilancia epidemiol\u00f3gica de la resistencia realizados en 8 hospitales de Espa\u00f1a y 11 hospitales de Portugal, con la participaci\u00f3n de los grupos del CIBERINFEC en el Servicio de Microbiolog\u00eda del Hospital Universitario Ram\u00f3n y Cajal-<a style=\"color: #000000;\" href=\"https:\/\/www.irycis.org\/es\/\" target=\"_blank\" rel=\"noopener noreferrer\">IRYCIS<\/a>\u00a0(centro coordinador del estudio),\u00a0<a style=\"color: #000000;\" href=\"https:\/\/xxicoruna.sergas.gal\/Paxinas\/web.aspx\" target=\"_blank\" rel=\"noopener noreferrer\">Hospital Universitario de A Coru\u00f1a<\/a>,\u00a0<a style=\"color: #000000;\" href=\"https:\/\/www.hospitalmacarena.es\/\" target=\"_blank\" rel=\"noopener noreferrer\">Hospital Universitario Virgen de la Macarena<\/a>\u00a0(<a style=\"color: #000000;\" href=\"https:\/\/www.csic.es\/es\/formacion-y-empleo\/formacion-de-personal-investigador\/master\/convenios-csic-universidades-master\" target=\"_blank\" rel=\"noopener noreferrer\">CSIC \/ Universidad de Sevilla<\/a>) y\u00a0<a style=\"color: #000000;\" href=\"https:\/\/www.hospitalsonespases.es\/\" target=\"_blank\" rel=\"noopener noreferrer\">Hospital Universitario Son Espases<\/a>\u00a0de Palma de Mallorca. Asimismo, tambi\u00e9n ha colaborado el Centro de Investigaci\u00f3n Biom\u00e9dica en Red de Enfermedades Respiratorias (<a style=\"color: #000000;\" href=\"https:\/\/www.ciberes.org\/\" target=\"_blank\" rel=\"noopener noreferrer\">CIBERES<\/a>) de Espa\u00f1a en el\u00a0<a style=\"color: #000000;\" href=\"https:\/\/www.comunidad.madrid\/hospital\/gregoriomaranon\/\" target=\"_blank\" rel=\"noopener noreferrer\">Hospital Gregorio Mara\u00f1\u00f3n<\/a>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Se ha estudiado la sensibilidad antibi\u00f3tica de un total de 747 enterobacterias y 474 P. aeruginosa procedentes de infecciones complicadas intraabdominales, del tracto urinario y del tracto respiratorio inferior de pacientes de UCI (Unidad de Cuidados Intensivos). Mediante secuenciaci\u00f3n de genoma completo se caracteriz\u00f3 un subgrupo de 199 aislados de Enterobacterales y 62 de P. aeruginosa con distintos fenotipos de sensibilidad a imipenem-relebactam.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">\u201cEl imipenem-relebactam mostr\u00f3 excelentes valores de sensibilidad tanto en enterobacterias (98,7%) como en P. aeruginosa (93,7%)\u201d asegura Marta Hern\u00e1ndez-Garc\u00eda, investigadora del CIBERINFEC en el Hospital Ram\u00f3n y Cajal y primera firmante de estos trabajos. El imipenem-relebactam mostr\u00f3 un 100% de sensibilidad frente a los aislados de K. pneumoniae y Escherichia coli productores de BLEE y un 80% frente aislados de K. pneumoniae productora de carbapenemasas.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Adem\u00e1s, el relebactam recuper\u00f3 la actividad del imipenem en el 77% de los aislados de P. aeruginosa, incluyendo cepas resistentes de dif\u00edcil tratamiento (RDT, 67%).<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Los investigadores destacan que &#8220;el relebactam recuper\u00f3 la actividad del imipenem en todos los aislados de Enterobacterales y P. aeruginosa productores de carbapenemasas de tipo KPC&#8221;. En Enterobacterales, la resistencia a imipenem-relebactam se asoci\u00f3 principalmente con clones de alto riesgo de K. pneumoniae predominantes en Espa\u00f1a, mientras que en la colecci\u00f3n de P. aeruginosa se asoci\u00f3 con la producci\u00f3n de GES-13 en el clon CC235 (en Portugal) y de enzimas de tipo VIM en el CC175 (en Espa\u00f1a).<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Marta Hern\u00e1ndez-Garc\u00eda se\u00f1ala que \u201ca pesar de la proximidad geogr\u00e1fica de ambos pa\u00edses, en los estudios SUPERIOR y STEP se observan diferencias en la distribuci\u00f3n de los mecanismos de resistencia en los aislados multirresistentes de Enterobacterales y P. aeruginosa de los pacientes ingresados en UCI, aspecto relevante a la hora de establecer estrategias de tratamiento y de contenci\u00f3n de la dispersi\u00f3n de las resistencias\u201d.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Adem\u00e1s, \u201cel imipenem-relebactam se presenta como una buena opci\u00f3n terap\u00e9utica en el tratamiento de las infecciones complicadas de dif\u00edcil tratamiento, incluidas las producidas por bacterias multirresistentes productoras de carbapenemasas de tipo KPC\u201d, concluye Rafael Cant\u00f3n, coordinador de ambos estudios y jefe de grupo del CIBERINFEC en el Hospital Universitario Ram\u00f3n y Cajal-IRYCIS.<\/span><\/p>\n<p>Fuente:<\/p>\n<p><i>Nueva combinaci\u00f3n eficaz de antibi\u00f3ticos contra bacterias multirresistentes | madrimasd<\/i>.
